In order to enroll a student into The Beverly Institute, the following steps must be completed:
There is a non-refundable application / registration fee of $ 75.00. (there is a $5.00 processing fee for all credit card transactions.) A student is considered registered upon payment and receipt of a completed registration forms. This payment is not applied to the first month’s tuition.

Copy of Birth Certificate and Social Security Card

Immunization Card (form 680)
Applicant Questionnaire - must be completed by student.
Essay Form - Essays may be submitted under separate cover.
Parent Questionnaire – Please return with application.
Request for records – signed by parent and given to the appropriate official at the student’s current school. Grades and test scores are to be sent to The Beverly Institute.
English Teacher Recommendation Form – to be sent by the school directly to The Beverly Institute
Math Teacher Recommendation Form – to be sent by the school directly to The Beverly Institute
Disciplinary / Character From – to be sent by the school directly to The Beverly Institute
Admission Form - signed and dated by parent and student.
Scholarship Award letter; if applicable.
Upon receiving all forms that apply to the perspective student, the school receptionist will contact the parent or guardian to schedule the initial interview for Individual academic assessment. Additional interviews may be scheduled following review of recommendation and conduct forms.
ADMISSIONS PROCEDURES: New Student Part Two:
Confirmation with the student’s current school that the necessary documents have been submitted to The Beverly Institute and all interviews are completed, then prospective student's enrollment information will be given to the Admission Committee for final review.
Admission decisions will be mailed with-in two weeks.
Students who are accepted will receive a acceptance letter stating that they have meet all TBI general admission requirements. Parents will then need to pick up and enrollment packet: TBI contract, authorized pick-up and dispensing of medication, School policies, and explanation of all fees.
Upon receiving a signed and notarized form the parent/ student will receive a welcome letter by mail, this letter will inform parent/student of important dates and fee schedules.
All fees must be paid before Orientation Night to receive 1st day of school pass.
