Eligibility
Enrollment Procedures for TBI
Registration Fee
There is a $ 100.00 registration fee. A student is considered registered upon payment and receipt of a completed
registration form. This is non-refundable and does not apply to the first month’s payment. This cost will hold your child’s
position, and will cover the cost of enrollment forms.
Complete and return all enrollment forms and record release forms. The following documents MUST be turned in with the
enrollment forms:
Enrollment Procedures for Nursery
- Pregnant (or suspects pregnancy) or parenting students
- Students must have documentation of pregnancy
- Parental consent if under 18 years of age
Enrollment Procedures for TBI

registration form. This is non-refundable and does not apply to the first month’s payment. This cost will hold your child’s
position, and will cover the cost of enrollment forms.
Complete and return all enrollment forms and record release forms. The following documents MUST be turned in with the
enrollment forms:
- Make an appointment for evaluation.
- Copy of the most recent and up to date report card (if enrolling for the fall, the reported card must be the final
report card from the previous school year) - Copy of the student’s unofficially transcript (for enrolling high school students only)
- Copies of the must recent standardized test results, (FCAT, Stanford, etc..
- Copies of birth certificate and up-to-date immunization card (form 680)
- Copy of Student’s Social Security Number
- Behavior Form (filled out by previous school)
- Court Order relating to student, if applicable (i.e.,) Power of Attorney, Guardianship, Custody, and etc..
- Scholarship Award Letter, if applicable
- Admissions Statement
Enrollment Procedures for Nursery
- Must submit baby's birth certificate
- Social Security Card for baby and immunization (HRS 680 card)
